## Steps
1. Placeholder scan: TBD, TODO, incomplete, vague — fix them.
2. Internal consistency: no contradictions; architecture matches features.
3. Scope check: one implementation plan, or decompose.
4. Ambiguity check: if a requirement can be read two ways, pick one and make it explicit.
